Our population Health Hubs increase proactive mental health engagement, improve resource navigation, and drive meaningful symptom improvement. By guiding people to the most appropriate support earlier in their journey, organizations reduce wait times, prevent escalation to higher-cost care, and improve the efficiency of their behavioral health system.

1. Reduce the strain on in-person and downstream services

Without clear, measurement-based service navigation, many people either do not access support or rely on high-cost in-person care instead of services that are most appropriate for their needs. In our implementation data, 28% of individuals would not have accessed any support at all without their Population Health Hub, while 18% would have relied on emergency or urgent care services. This creates unnecessary pressure on in-person providers and increases overall care costs. By guiding people to appropriate upstream resources such as self-guided CBT, group therapy, and other digital supports, the Hub helps individuals access the appropriate level of care earlier in their mental health journey. This reduces reliance on crisis services, shortens wait times for higher-acuity care, and ensures in-person services remain available for those who need them most.

2. Fulfill the need for low-cost, self-guided resources

Usage data from our Population Health Hubs show that when appropriate self-guided resources are available, 70% of individuals choose these options first rather than accessing 1:1 clinical services. By recommending supports based on acuity, conditions, demographics, and individual preferences, the Hub helps ensure people begin with the most appropriate level of care for their unique needs. This significantly reduces demand for limited clinical capacity while still ensuring meaningful support and improvement, supported by individuals using self-guided supports through the Hub showing an average symptom improvement of 19% without accessing clinical care.

Improve population-level outcomes

Engagement with outcomes tracking drives measurable symptom improvement. Our Population Health Hub data shows that regardless of the resources individuals access, or whether they access resources at all, the more assessments they complete over time, the greater their symptom improvement. This reflects the same evidence behind Measurement-Based Care in clinical settings. Regularly tracking mental health changes helps individuals better understand their mental health and improve their symptoms. By embedding outcome tracking directly within the Population Health Hub, organizations can extend the benefits of Measurement-Based Care beyond clinical settings and drive meaningful improvement across an entire population.

Effective mental health support reduces the cost of physical healthcare

Without early mental health support, individuals are more likely to experience worsening symptoms that increase the use of both behavioral and physical healthcare services. Mental health and physical health are deeply connected, and untreated mental health challenges significantly increase total healthcare costs. Research shows untreated mental health conditions increase physical healthcare costs by about 60% compared to individuals with the same physical conditions who receive evidence-based mental health support (Clark & Layard, 2014). Additionally, about 40% of people living with anxiety or depression also have a long-term physical health condition, and the cost of managing those conditions is roughly 50% higher than for individuals with similar physical conditions who do not experience mental health challenges.

How often can users track their progress?

Users can track their progress by completing a brief wellness self-assessment, called a “check-in,” at regular intervals. The default cadence is every two weeks, which aligns with common Measurement-Based Care practices for monitoring changes in symptoms and well-being over time. Users can choose to receive reminders to complete a check-in every one, two, four, or eight weeks depending on their preferences. Organizations can also configure check-in frequency based on their population, program goals, or clinical best practices. The specific set of assessments included in a check-in is determined by the implementing organization or health system.

Assessments are selected based on the needs of the population being supported and the outcomes the organization is seeking to impact and monitor. These regular check-ins allow individuals to better understand their mental health over time,  inform recommended resources that are most appropriate for their needs and help organizations understand population-level trends and ensure people receive the right support at the right time.

How do regular assessments help to enhance user engagement and improve mental health symptoms?

Regular self-assessments play an important role in keeping individuals engaged with their mental health and helping them track meaningful changes over time. Within our Population Health Hub implementations, these assessments, called “check-ins,” provide individuals with a simple way to reflect on how they are doing and see their progress over time. This approach is grounded in the principles of Measurement-Based Care, where regularly capturing outcomes helps individuals better understand their mental health while enabling earlier identification of changes that may require additional support.

Across our hubs, the average 30-day user retention rate is 43%, which is significantly higher than the typical 3–6% retention rate seen across most self-guided mental health tools. Usage data shows that these regular check-ins are a primary driver of continued engagement. In fact, 93% of returning users come back specifically to complete their check-in after receiving reminder notifications via email or SMS.

This pattern suggests that users return not only to access resources, but to better understand their own mental health and track their progress over time. These regular moments of reflection help keep individuals engaged in preventative mental health care.

Assessment engagement is also strongly associated with improved outcomes. Based on data from more than 600,000 users, individuals who complete check-ins more consistently show greater improvements in their mental health symptoms over time.

How can Hub users interpret their data? Are there resources to support them?

The Population Health Hub presents users with non-diagnostic mental health insights based on their check-in responses, helping them understand how their well-being is changing over time. Rather than labeling or diagnosing symptoms, the Hub highlights trends and changes since a user’s previous check-in and provides reflection prompts that encourage users to consider what may be influencing their mental health.

Completing check-ins over time unlocks automated insights that help users identify meaningful changes in their results and suggest potential actions that may support continued positive progress. These insights help users better understand patterns in their mental health and reflect on what may be contributing to improvements or challenges in their day-to-day lives.

This approach reflects the principles of Measurement-Based Care, where regularly capturing outcomes helps individuals build awareness of how their symptoms evolve and what factors may be positively or negatively affecting their well-being. To support this process, the Hub recommends relevant resources based on a user’s most recent check-in results and needs. These may include self-guided supports, community resources, or connections to professional care.

At the same time, the platform enables implementing organizations and health systems to better understand population-level needs and outcomes trends. This allows leaders to identify where support is working, where gaps may exist, and how to continuously improve the care and resources available to their communities.

How will clinicians and clinical teams be able to leverage data to support users?

Your Population Health Hub will be configured based on your organization’s specific implementation. Depending on how the Hub is deployed, there are three ways clinicians and care teams can leverage the data generated through user check-ins.

User-led sharing of results

When the Hub is implemented as a standalone resource, individual check-in results remain private to the user and are treated as personal health information. In this case, users can choose to export their results and bring them to counselling sessions or share them directly with a clinician. This allows their progress data to inform conversations in care, helping clinicians better understand symptom changes over time.

Organizations operating the Hub can still access aggregate, deidentified insights through their population dashboard. This provides visibility into trends across the community while maintaining individual privacy.

Integrated Measurement-Based Care

For organizations implementing the Hub alongside Greenspace’s Measurement-Based Care platform or Intake solution, user check-in data can be integrated directly into clinical workflows. With user permission, clinicians and care teams are able to view outcomes data within Greenspace or within their electronic health record system, allowing results to inform treatment planning, progress monitoring, and care decisions.

EHR Integration

If the Hub is deployed independently but your organization stores outcomes within an electronic health record system, user results can also be synced to the client’s record, with their permission. This allows clinicians to incorporate check-in data into treatment goals, progress discussions, and ongoing clinical decision-making.

Across all implementations, the goal is the same: enabling individuals to better understand their mental health while ensuring clinicians and organizations have access to meaningful insights that support more responsive, outcomes-informed care.

Is there an option for users to access the Hub from a mobile application?

The Population Health Hub is accessible from any device through a mobile, tablet, or desktop browser, and has been optimized to provide a seamless experience across all screen sizes.

For organizations that want to further increase accessibility and engagement, Greenspace also offers the option of a dedicated mobile app. The app can make it easier for users to complete check-ins, access resources, and stay connected to their mental health over time, helping organizations reach users where they already spend much of their time. This flexibility allows each organization to determine the access experience that best supports their population and engagement goals.

Does Greenspace maintain security practices in line with industry best practices and US standards?

Security of patient information and privacy is the foundation of the Greenspace platform. We take this responsibility very seriously and have implemented significant measures to safeguard patients’ personal health information that exceed industry standards and best practices. Greenspace is SOC 2 Type II compliant and conforms to digital and physical security protocols (including HIPAA), with SSL-secured access, AES encryption at the filesystem level, and firewalls protecting all data. We take many additional precautions to protect privacy including: requiring strong passwords, automatic logouts, automatic access logging, secured data backups, two factor authentication and restrictive data access procedures. Who can see users’ personal information?

The information users provide on this site belongs to them and is kept private and confidential. Unless otherwise agreed to by the user for research or clinical purposes, personal information will not be shared, sold, or disclosed to third parties, as described in our Privacy Policy. Is it secure for users to complete assessments online?

Yes. Security and privacy are core priorities in the design of the Population Health Hub.

Assessments delivered through the Hub, whether accessed via browser, email, or SMS reminder links, do not contain personally identifying information within the assessment itself. This helps protect user privacy throughout the process.

When assessments are completed, responses are transmitted to the platform’s servers through secure encrypted channels such as HTTPS. No user information in conjunction with user names is ever sent over unsecured email or other unsecured channels.

Greenspace meets or exceeds the standard security and privacy requirements set by HIPAA and is SOC 2 Type II compliant. These safeguards help ensure that individuals can safely complete check-ins and engage with the Hub while their information remains protected.
