Grand River Hospital\u2019s mental health &amp; addictions programs support individuals of all ages and care needs as they continually strive to positively impact the mental health of their community.<\/p>\n<p>Grand River Hospital recognizes that efficient and consistent tracking of patient symptoms throughout care will have a meaningful influence on improved discharge planning, safe transfer of care to community-based providers, and positive treatment outcomes.
